We will always try to ask the family’s to be actively involved in the writing of support plans. This has proved to have a positive effect on all people involved, aiding the progression of individuals. Continuity of support is improved when the approaches identified in the Person’s Support Plan have then been continued by the family during short breaks taken with them.
Developing activities and interests is an on-going process, and changes as people grow. This is an area which is reviewed and developed on a regular basis, within monthly meetings with the individual and their identified keyworker.
